Postarthroscopic Glenohumeral Chondrolysis (PAGCL) is a horribly painful and debilitating condition involving cartilage loss in the shoulder after arthroscopic surgery. It is a relatively new condition/disease, and the cause is still under investigation. So far, the one consistent factor in PAGCL is the use of a pain pump immediately after surgery.

A pain pump delivers pain medication directly to the surgery site through a catheter. Marcaine (Bupivacaine), Epinephrine or Xylocaine (Lidocaine) are the medications typically used in the pump after shoulder arthroscopy. The disposable pump is only used for two or three days, and the catheter can be removed by the patient, once it is no longer needed.

At one time it was suspected that the use of thermal devices during arthroscopy could play a role in the development of PAGCL. However, researchers have found that some patients developed PAGCL without the use of thermal devices. All of the patients with PAGCL used the pain pump.

It is important to note that the pain pump is not the sole means of pain relief in these surgeries. The pump is used in addition to oral pain medications such as hydrocodone, Demarol, or morphine. Because the pain pump is the one known consistent factor in PAGCL, some researchers have recommended that doctors stop using it in shoulder arthroscopy until more can be learned about the condition.
