Postarthroscopic Glenohumeral Chondrolysis (PAGCL) is the deterioration or loss of cartilage in the shoulder following shoulder arthroscopy. It is painful, debilitating, and can be life altering. Your doctor may begin treatment by trying anti-inflammatory medications. However, it is not uncommon for people with PAGCL to require shoulder joint replacement.

Shoulder joint replacement (arthroplasty)
Arthroplasty replaces your shoulder joint with artificial parts, made of plastic and metal. Full shoulder replacement surgery involves replacing the cuplike bone in front of the shoulder blade, called the glenoid, and the top of your arm bone, called the humeral head.

Your original shoulder arthroscopy may have been an outpatient procedure or “day surgery.” Arthroscopy is minimally invasive, and can sometimes be performed with local anesthesia. Shoulder replacement is a much more extensive procedure, requiring general anesthesia, and may require one to three nights in the hospital. The surgery itself takes two to three hours to perform.

Recovery
After your surgery you will require physical therapy. You will see a physical therapist and have exercises to perform at home. Rehabilitation can take several months, including two or three physical therapy sessions a week for the first six weeks.

Complications
Like any surgery, shoulder replacement surgery carries certain risks. Complications can include:

  • Anesthesia complications
  • Infection immediately after the procedure
  • Nerve injury
  • Blood vessel injury
  • Infection which does not reveal symptoms for months or years
  • Infection which spread to the joint from other parts of the body, after other procedures, such as dental work
  • Dislocation
  • Loosening of the artificial joint from the natural bone
