What is Postarthroscopic Glenohumeral Chondrolysis (PAGCL)?
PAGCL is cartilage loss that usually begins to appear months after arthroscopic shoulder surgery. It is a devastating complication of arthroscopic shoulder surgery, which causes pain, reduced range of motion and sometimes the inability to use the affected shoulder.
What causes PAGCL?
PAGCL is a relatively new condition, and research as to the cause is still underway. It is believed to be caused by the use of a pain pump immediately after arthroscopic shoulder surgery. The pain pump delivers Marcaine (Bupivacaine), Xylocaine (Lidocaine) or Epinephrine directly into the surgical site for the first two or three days after surgery.
What are the symptoms of PAGCL?
The symptoms of PAGCL are often similar to the symptoms that surgery was intended to relieve, and can be much worse.
- Shoulder pain both at rest and in motion
- Stiffness of the shoulder
- Weakness of the shoulder
- Clicking, popping, and/or grinding
- Limited range of motion
- Narrowing joint space, visible on x-ray
How is PAGCL treated?
Some patients, with mild PAGCL may benefit from the use of anti-inflammatory medication. Those who have lost all cartilage require arthroplasty, shoulder replacement.
Can PAGCL affect other parts of the body?
Yes. Postarthroscopic chondrolysis, or post-operative cartilage loss, can affect other joints in the body (glenohumeral refers specifically to the shoulder joint) including:
- Knee
- Ankle
- Hip
Who gets PAGCL?
Anyone who has had shoulder arthroscopy can develop this devastating complication. Researchers are particularly concerned about the fact that it occurs in young patients after arthroscopy using a pain pump, because cartilage loss is typically more common in older people.
